Proctor Form

Please follow the instructions give below to find a proctor:

If you live around Barstow area, you must take your final exam in the Barstow College Computer Lab. If you live at Ft Irwin, you must take your exam at Barstow College, Ft Irwin site. You don't need to submit this form if you are taking your exam at Barstow College or Ft Irwin Site.

If you live outside Barstow area and Ft. Irwin, you are required to find a proctor. Please read the proctor qualification guideline given in the syllabus to find a proctor. Note: The proctor cannot be related to you, a friend, or an employer. The exam must be proctored in the location where the proctor is employed. The day and the time of the scheduled proctor exam must be e-mailed to me prior taking the exam including the site telephone number. The final exam will be administered during the ninth week of instruction. The final will be given in MyMathLab between 3/7 - 3/10.

Important note: If you have used some other proctors in the past, who does not have the proctor qualifications given in the syllabus, you will be asked to follow the above instructions to find a proctor for this course, NO EXCEPTIONS.

The deadline to submit the proctor information is 3/2. Failure to submit the proctor info will require the student to take the final at Barstow Community College computer lab.

Complete the box by entering your name, email address, postal address and phone number. Send the information to your instructor by clicking on the "Submit Answers" button at the end of the proctor form.

ATTENTION: This is a form to be filled out by students who require proctoring for their tests. If you are able to come to the scheduled exams you do not have to fill out this form.

In addition filling out this form does not automatically mean your proctor has been accepted. The proctor will still need to receive instructor approval.

Finally please remember that the top box is for YOUR information and the bottom box is for your PROCTOR'S information. A proctor for this class must be unrelated to you and be approved by the instructor. The college is not responsible for proctoring fees.
