Proctor Form

Attention all students:  Having a proctor is a requirement to offer this course. It is imperative that you secure a proctor and submit a completed proctor form within the required time frame.

You have until October 24, 2010 to submit a Proctor Form.  If you do not have a completed Proctor Form submitted within the given time frame, you will be administratively dropped from the course at the discretion of the Instructor.

A Proctor is used so that the instructor can communicate with a third party to confirm that the student is participating in his/her chosen activity for the required minimum of five hours per week.

A proctor can be a family member, co-worker, instructor, etc.

If you have any questions regarding getting a Proctor or completing the Proctor Form, please contact your instructor immediately at dgrossma@barstow.edu.

DO NOT POST AHEAD OF THE ASSIGNED LESSON OR DISCUSSION QUESTION!

Do not submit your required exercise/workout log until you have completed the minimum requirement of 5 hours of activity per lesson.

I will not accept any work that is done in advance. All work that is submitted early will be deleted.  You will not get credit for the work until it is submitted when due.

Check Your Progress - Class is over. Your final grade will be calculated and submitted to Admissions and Records by Monday December 20, 2010.

This page will be where you can track your progress throughout the nine (9) weeks of the class.

Each assignment is worth 10 points

Each Discussion Post is worth 10 points

Each Activity Log is worth 50 points

If you are not receiving full credit for your Activity Logs, please review the Grading Rubric for Activity Log.

In addition, if you do not proofread your work before you submit it and there are spelling and grammatical errors, you will lose points.  Also, if you submit your work as if you are texting, you will lose points.
